What to Bring to Your First Appointment

Bringing the following items helps us prepare for your consultation and ensures we can create an accurate, customized treatment plan:

  • Referral form from your dentist or healthcare provider (if applicable)
  • Recent dental X-rays (if available)
  • A list of medications you are currently taking
  • Photo ID
  • Dental and medical insurance cards
  • Surgical or medical history information
  • A parent or guardian for patients under 18

What to Expect During Your First Visit

Your first appointment typically includes:

1. Comprehensive Consultation

You will meet with one of our board-certified oral and maxillofacial surgeons—Dr. Maher Jandali or Dr. John Hur—who will review your medical history, symptoms, and goals. We will discuss your treatment options, answer your questions, and explain every step clearly.

2. Diagnostic Evaluation

Most patients undergo an examination and advanced imaging so we can accurately diagnose your condition and plan treatment with precision.

3. Customized Treatment Plan

You will receive a clear, detailed treatment plan outlining recommended procedures, anesthesia options, expected healing time, and follow-up care.

4. Scheduling & Pre-Operative Instructions

Our team will help you schedule your procedure and provide any necessary pre-operative guidelines to ensure a safe, smooth experience.

Many simple procedures may be performed the same day as your consultation, depending on your medical history and level of surgical complexity.

3D Imaging for Precise Diagnosis

We use state-of-the-art 3D CBCT imaging to evaluate bone structure, tooth position, impacted teeth, jaw alignment, and other anatomical details that traditional X-rays cannot show. This technology allows our surgeons to diagnose conditions with exceptional accuracy and plan treatments that prioritize safety, predictability, and long-term success.

3D imaging is commonly used for:

  • Oral pathology evaluation

X-Guide / Surgical Navigation for Implant Precision

For implant procedures, our practice utilizes advanced X-Guide surgical navigation technology. This dynamic, computer-assisted system enhances precision during implant placement by providing real-time imaging and guidance. X-Guide helps achieve the ideal angle, depth, and position for each implant, improving accuracy, minimizing complications, and supporting long-lasting results.

Financial & Insurance Information

At Oral and Facial Surgery of Naperville, we work hard to help you understand and maximize your dental and medical benefits. When full insurance information is provided, our team can submit claims on your behalf and guide you through expected coverage.

Insurance Overview

  • Insurance is a contract between you and your carrier, and coverage varies by plan.
  • Our fees fall within the usual and customary range used by most insurers.
  • Some procedures—such as dental implants and certain extractions—may not be covered by insurance.
  • Completely impacted teeth may qualify under medical insurance depending on the plan.
  • We are not enrolled with Medicare, and Medicare does not cover tooth extraction.
  • We are not contracted with Dental DMO or HMO plans, so claims cannot be submitted to those plans.

Plans We Participate With

Delta Dental Premier

We are in-network with Delta Dental Premier. All other dental plans are considered out-of-network, but we can file claims for you.

Illinois Health Partners (IHP)

For patients with IHP medical plans, a referral from your primary care physician is required for coverage. We can evaluate whether your procedure may qualify under your medical benefits.

Payment & Coverage Expectations

  • When full insurance information is provided, half of the surgical fee is due at the time of service.
  • After we file a claim, the insurance company will send payment directly to our office.
  • You are responsible for any remaining balance after insurance processes the claim.
  • Insurers typically respond within 4–6 weeks. If no payment is received within 60 days, the balance becomes your responsibility.
  • Refunds for overpayments are issued promptly, and remaining balances are billed to the patient.

Patients Without Insurance

Patients without dental insurance must pay in full at the time of service.

CareCredit Financing

Available for bills over $300, including:

  • Flexible monthly payments
  • 6-month interest-free options

Medical Insurance Notes

  • Most medical insurance plans do not cover dental extractions or implants.
  • Orthognathic (jaw) surgery coverage is determined by medical necessity and your specific plan.

HMO Medical Plans (IHP Only)

  • Requires prior authorization before surgery.
  • Co-pays may apply.
  • Some procedures are fully covered; others are not.
  • If not covered, the patient is responsible for the full cost.

Cancellation Policy

  • Extractions: 24-hour cancellation notice
  • Implants: 72-hour cancellation notice. A fee may apply if proper notice is not given.
